For Membership
Membership Information:
1. Print out and complete (in writing) the CMAANL Application
forms and Code of Ethics forms. See bottom of this page for link to these
Printable Documents.
2. Applicants must send their completed application forms to the CMAANL office. Applicants will be contacted for
an interview with the Chapter President .before the forms are sent to
the CMAAC Office for final approval.
3. Upon chapter approval, their forms will be forwarded to the CMAAC Head office for final approval as dual mebership is required (CMAAC and CMAANL)..
All completed application forms must include the following:
o Application / Processing fee of $200 (non-refundable) for membership to the CMAANL
o Application / Processing fee of $240 (non-refundable) for membership to the CMAAC
o Two (2) Passport-sized photos (must be signed by an Accredited Guarantor,
i.e., Notary Public)
o Certified copies of all Academic and Clinical credentials
o Copies of Education Transcripts
o Proof of Landed Immigrant Status (if applicable)
o 2 Letters of Reference
o Signed Code of Ethics and Responsibilities
- Only applications from qualified professionals will be considered. (Please
see CMAANL’s Standards for Membership for more details)
- Completion of an oral, written, or practical examination may be required
in addition to the presentation of credentials.
- Successful applicants will receive a membership certificate.
- Income tax receipts will be issued for successful applicants.
